Pre-participation Physical Examination

  1. Albright College requires all freshmen and transfer students to undergo a physical examination completed by their family physician prior to competing in intercollegiate athletics. The cost of the examination is the responsibility of the student-athlete. The form should be completed and returned to the Athletic Training Room .   2. Each student-athletes is also required to complete an extensive health history questionnaire and insurance form prior to participation annually. These forms must be completed accurately. Failure to inform the staff of a pre-existing condition or injury will relieve Albright College of any liability relative to this condition. These forms must be completed and returned to the Athletic Training Room.

  3. If the student- athlete is currently under the care a physician for any injury / illness the student- athlete must provide written documentation from their physician regarding their ability to participate in intercollegiate athletics. For all orthopedic injuries the final decision regarding participation will be made by the Albright College Team Physician and / the appropriate specialist.
